It keeps coming loose and the rim clunks against the brake hub.  I have to jack up the car and reset the rim to the hub and lugs.  and tighten.  This time I had BRett,   press the brake pedal and hold the wheels while I tightend the lugs real tight with the  car on the jack.  Thee tighten again when the wheel was on the ground.  Maybe this will stay stuck

Your wheel spacers are not hub or even bolt centric for that matter. That is causing a wobble which is causing your nuts to loosen. Now if you keep letting them loosen up you will wallow out your wheels, and or break a stud off the hub.
